Data Reporting Requirements

As of 2018 the Baker Act Reporting Center (BARC) receives mental health documents that pertain to longer term placement for individuals initially bought to a receiving facility under the FL Mental Health Act (Baker Act). BARC works with county Clerks of Court to receive the following documents as per and Advisory 18-009:

  • Petition for Involuntary Inpatient Placement (DCF suggested form ) [394.467(3)]
  • Petition for Involuntary Outpatient Placement/Outpatient Services (suggested form ) [394.4655(4)(c)]
  • Certificate & Continued Individualized Treatment Plan of Outpatient Treatment (suggested form ) [394.4655(8)(a)4]
  • Order for Involuntary Inpatient Placement (suggested form ) [394.463(2)(e)]
  • Order for Involuntary Outpatient Placement/Outpatient Services (suggested form ) [394.463(2)(e)]
